After careful deliberation, we are excited to announce that SHCC will transition to a membership practice model starting October 1st, 2024.
Our commitment has always been to provide the highest quality care by maintaining our independence. However, the evolving healthcare landscape in America has posed challenges, including reduced reimbursements and an increase in uncompensated services for independent providers.
Membership practices have become more common and successful in our community, enhancing satisfaction for both patients and providers. This model will allow us to offer services and care beyond what traditional insurance covers, ensuring comprehensive and personalized attention.
Key Points to Note:
Insurance: Your current insurance and billing procedures, including copays and dedctibles, will remain unchanged. The services covered by your insurance will not be affected.
Membership Benefits: Membership is separate from your insurance and includes additional services aimed at improving your healthcare experience, such as same-day appointments, addressing multiple issues in one visit, and ensuring our healthcare professionals can dedicate ample time to your concerns.
We hope you share our vision for what membership care can achieve: personalized, accessible and independent healthcare free from the constraints of larger health corporations. We understand that a membership model may not suit everyone. We aim to give you ample time to consider this change and, if necessary, find a new provider. Annual Membership Fees:
Adults (18 years and older): $240
Children (0-17 years of age): $120
To enroll, please complete and return the Membership Services Agreement, including your payment method. You can also complete the agreement on our website. Memberships will automatically renew annually.
