Plan ahead and bring the following items with you: Insurance card, Photo ID, Co-Pay, Previous Medical Records (if applicable), & a list of medications (including herbal and naturopathic).

If you are coming in for a Medicare Annual Wellness Visit (AKA Medicare Health Risk Assessment) then please fill out This Form.
All New Patients must also fill out and review the following:

- Adult Health History
- Pediatric Health History (Ages 17 and below)
- Notice of Privacy Practices

Getting the Most from Your Medical Insurance
Ask these simple questions to help you get the most out of your insurance benefits.

Is the Service Covered?
Does the insurance plan cover the service you require? I.e. are preventative services covered, and if so, is there a dollar limitation?

What will I owe?
Do I have a co-payment? Do I have a deductible? Following my deductible, how much will I owe?

What do I need to do?
Do I need a referral of preauthorization? Did I remember to bring my insurance card or information?
Billing Contracted Insurance
As a courtesy, we will bill all insurance carriers. Co-payments are due at the time of service. We will await payment from contracted carriers before balance billing. Your statement will reflect any processing of the claim(s). Statement patient totals will reflect the amount currently outstanding and are the patient responsibility. All patient balances are due and payable within 25 days of your first statement unless you arrange an extended payment plan with our billing department. A $10.00 rebilling fee may be added to overdue accounts. Returned NSF checks will be charged a $35.00 handling fee.
Billing Non-Contracted Insurance
As a courtesy, we will bill all insurance carriers. Co-payments are due at the time of service. If we are submitting a courtesy bill to a non-contracted insurance carrier, we will wait 60-days for the claim to be processed before we send out a patient statement. Patient statements will reflect the amount currently outstanding and are the patient’s responsibility. All patient balances are due within 25 days of your first statement unless you have pre-arranged an extended payment plan with our billing department. A $10.00 rebilling fee may be added to overdue accounts. Returned NSF checks will be charged a $35.00 handling fee.
Private Pay
If you do not have medical insurance, we require a minimum of $150.00 at your visit. We offer a 20% discount of the charges if paid in full at the time of service. All balances are due and payable within 25 days of your first statement, unless you have arranged an extended payment plan with our billing department. A $10.00 rebilling fee may be added to overdue accounts. Returned NSF checks will be charged a $35.00 handling fee.
For your convenience we accept cash (please do not send cash through the mail), check, money orders, Discover, MasterCard or Visa.
Financial Assistance
We understand that there are circumstances that may warrant an extended payment plan. To request special consideration, please call our billing department at (360) 876-2434. We will work with you to find a satisfactory resolution to your situation.
Statements
Your account has been set up either as an individual account or as a family account according to the account guarantor information you provided on your initial registration form. A household may have more than one guarantor. This will produce multiple statements. If you prefer just one statement for all members of your family, please be sure to select the same guarantor for all members of the family. Please remember that you must be at least 18 years of age to qualify as an account guarantor.
